MEMBERSHIP AGREEMENT

I understand that membership is for me, my spouse, and children living in my home. If there is a parent who lives in a different household, they are required to apply for their own membership. 9. I hereby understand the terms of membership in CCH and agree to follow these guidelines for the entire duration of my membership. I also understand that failure to comply with these terms at any time could result in membership termination, and that this form is binding for as many years as I choose to re-enroll.

Payment Instructions

**Please note that although the dues payment option will appear at the end of the application, you will be unable to pay until you have completed the entire application and interview process. Once your membership has been approved, you will be invoiced for your dues and can pay at that time.**

**Dues must be paid within one week of receipt of invoice.**

As a reminder, CCH only accepts digital payments for annual dues.
